4. Photocopy of Certificate of Employment and Service Record from previous employers (if any)
5. Photocopy of Transcript of Records and Diploma/Certification of Units earned, if applicable
6. Photocopy of Certificate of Eligibility/Board Rating/PRC License (if there are)
7. Photocopy of certificates of trainings/seminars attended, awards received and similar undertakings (if there are)
8. Photocopy of latest performance rating (if any)
